New Service

Complete these forms when signing up for service. The MSA is required for all customers. The Term Service Agreement is required only if selecting a term plan.

Complete this form to open a new account. Includes business information, authorized users, package selection, DID/E911 setup, account configuration options, and payment method. First month's charges are due in full before service activation.

Master Service Agreement (MSA)

The governing agreement covering all services, billing policies, acceptable use, E911 requirements, FCC compliance (CPNI, STIR/SHAKEN, Robocall Mitigation), late fees, suspension, termination, and refund policy. Must be signed before service is activated.

Required for 12, 24, or 36-month term agreements. Documents your guaranteed package price, included minutes, NRC waivers, upgrade/downgrade rights, and Early Termination Fee (ETF) terms. Sign alongside the MSA and Account Setup Form.

Documents Provider's specific data handling commitments for customers with HIPAA or GDPR obligations. Covers CDR protection, call recording privacy, legal request procedures, and Customer's own compliance obligations. This addendum does not constitute a full BAA — consult your legal counsel. Must be signed by both parties.

Number Port Request

Port Request Form (LNP)

Complete this form to port (transfer) your existing phone number(s) to ZEGIT Tech Solutions VoIP Services. You must have a signed invoice from your current carrier ready. Porting can take from a few days to several weeks. A $8.75/number fee applies to each rejected port due to inaccurate information. Read all instructions on the form carefully before signing.

SMS/MMS Business Messaging

Required for any business that wants to send or receive text messages on their DID numbers. All business SMS/MMS traffic over US 10-digit numbers requires 10DLC registration with The Campaign Registry (TCR). Unregistered outbound SMS has been blocked by all major US carriers since February 1, 2025.

SMS requires separate 10DLC registration. You cannot send business text messages to US numbers without completing Brand and Campaign registration. This process takes 1–10 business days. Plan ahead before launch.

Complete this form if you want to enable SMS or MMS on any of your DID numbers. Includes Brand registration info (submitted to The Campaign Registry), Campaign registration, opt-in consent documentation requirements, and compliance acknowledgments. A professional business email address is required — Gmail/Yahoo/Hotmail will cause rejection. Resellers may submit on behalf of clients.
